The Community Habilitation Specialist (CHS) will provide social skills training to children/adolescent with Developmental Disabilities as classified by the OPWDD. Sessions take place in the community or in the home depending on the need of the child/adolescent. 

Salary Range: $17.22-$18.02\hr


-The Community Habilitation Specialist displays a congenial, warm acceptance for all individuals.
-The Community Habilitation Specialist will primarily be responsible for the assessment and
observation of the person’s skills development. Is alert to the expressions of attitudes, feelings and relationships of each individual. Develops an awareness of the individual’s needs, not only through the observation and interaction but through a
study of the individual’s case history and by appropriate consultation with other members of the
staff.
-Support the person in navigating the program and identifying incentives in learning new skills and
behaviors.
-Engage primary caregivers and other key participants in active change-oriented treatment by
identifying and overcoming barriers to engagement.
-Implement problem conceptualization, treatment planning, intervention implementation, and
outcome review and strategy revision procedure using the model's Analytic process (Brief Family
Systems Therapy and Parental Management Training).
-Develop Valued Outcomes with the steps on how to achieve it based on caregivers/community
supports and behaviors observed along with child/adolescent strengths.
-Community Habilitation Specialist meet with the assigned children/adolescent for 1-2 times per
week depending on the child/adolescent coordinated hours
-Community Habilitation Specialist will travel to borough assigned
-Community Habilitation Specialist attends supervision and training as required.
 It is essential that Community Habilitation Specialist know and protect individual’s rights.
 -Community Habilitation Specialist will document each training session in the appropriate databases
and report the progress and barriers with each client.
-The Community Habilitation Specialist will receive consultation and supervision from the
Community Habilitation Supervisor.
 -Positive and strength focused approach to cases

 

 

Salary Range

$17.22-$18.02

 

 

Educational Requirements:

BA/BS Degree;  Enrolled in Bachelor’s program or received Bachelor’s degree in Psychology, Social Work, Counseling or a
related subject area. Person must have significant clinical experience with the Intellectually/Developmentally
Disabled population may also qualify at the discretion of the agency.
Bilingual a plus, not required.


Other Requirements:
-Interest and/or experience in working with people with Developmental Disabilities
-Ability to observe a youth’s behavior and implement a behavior management program
-Demonstrate conceptual understanding of developmentally appropriate behavior for children/adolescents in the
home, school and community
-Ability to establish a strong relationship with a child/adolescent that combines warmth and humor with effective
skills
-Proficient in computer applications
-A clean and valid NYS driver’s license (preferred).
-Maintain all certifications. Including but not limited to the following: First Aid, CPR, and SCIP.
