1. Opens and sorts incoming mail.

2. Obtains hospital discharge records and uses hospital discharge list to reconcile receipt of paper charts.

3. Prepares the paper medical record for scanning.

4. Scans and indexes paper documents into the EMR.

5. Electronically analyzes, reanalyzes, and assigns signature deficiencies to the appropriate practitioner in the Chart Completion Module.

6. Performs quality reviews of scanned and interfaced images in the EMR and verifies the quality of image, accuracy of patient name, appropriate bar code, account number and document type.

7. Resolves issues with scanned and interfaced documents.

8. Maintains Turn-Around-Time (TAT) for prepping, scanning, analysis, and quality control of documents.

9. Sorts and scans loose sheets into EMR accurately daily.

10. Retrieves, signs out, and prepares medical records as requested for patient care, correspondence, studies, or audits.

11. Performs file maintenance and prepares records for offsite storage.

12. Responds to requests for patient information in a timely manner.

13. Communicate with internal and external customers requesting patient information.

14. Processes and files Advanced Healthcare Directives in accordance with Federal and/or State Statutes and Hospital/Departmental procedures.

15. Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers.

16. Performs other job-related duties and assignments as requested.

* EMRS Trainee - Grade S7 Job Code 8197
* Ability to read, write and communicate in English.
* High School Diploma or GED.
* Upon completion of the CCH EMRS Training program, the incumbent must pass the basic Medical Terminology course or pass the CCH Medical Terminology exam.
* Excellent customer service skills with the ability to communicate efficiently.
* Upon completion of the CCH EMRS training program, the incumbent must pass the Cape Cod Hospital Medical Record Department Qualifying test.
* With the successful completion of the CCH EMRS Training program, incumbent will transfer into EMRS Grade S8, Job Code: 8224.
* EMRS - Grade 8 - Job Code 8224
* Ability to read, write, and communicate in English.
* High School Diploma or GED.
* 12 months experience utilizing an Electronic Medical Records (EMR) system OR. successful passage of the Medical Record Department Qualifying test.
* Successful passage of basic Medical Terminology course OR successful passage of the CCH Medical Terminology exam.
* Basic knowledge of Microsoft Word, Excel, and Outlook. Excellent customer service skills with the ability to communicate efficiently.
* Exceptional organizational skills with attention to detail. Ability to problem solve, organize, and prioritize workload to meet turn-around time benchmarks.
