* Pay Range: $65,000 - $90,000

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life.

Our ideal candidate should possess a high level of energy and motivation, coupled with strong managerial, organizational, and interpersonal skills. They must be capable of providing precise and effective guidance and support, even in demanding situations, in order to enhance learning opportunities and foster professional growth.

Responsibilities

* Meeting the healthcare needs of hospice patients by completing as needed and routine visits
* Working closely with the Hospice team, physicians and community partners in order to deliver optimal care.
* Triaging any emergent needs and collaborating with physicians to provide comfort and quality care to each patient.
* Communicating with medical professionals, the hospice team and others
* Maintaining accurate, up-to-date records
* Providing services in accordance with agency policies and regulations that define scope of practice.
* Participating in in-service programs
* Demonstrating strong communication and customer service skills

Requirements

* Missouri Nursing License
* Valid driver's license
