Responsibilities

Position Summary: Community Health Education Coordinator

The Community Health Education Coordinator is responsible for coordinating and delivering community health education programs as part of St. Josephs Community Health department. The Coordinator promotes organizes and conducts community outreach and education classes health fairs and other events in the hospitals community benefits service area. The coordinator prepares and delivers presentations that meet the goals and objectives for various programs. Services are provided to a diverse population and the work includes outreach in low-income and undeserved communities. While conducting outreach and education the Coordinator also evaluates health related needs and assists community members to access appropriate health-related programs. In order to engage SJMC patients in community health programs that will support their recovery the Coordinator will also engage with patients while in the hospital and follow-up with them post-discharge.

The Coordinator must maintain positive interaction with Medical Center departments staff volunteers and community members. The Coordinator will also collaborate with community based organizations to outreach and provide health education programs. For community based organizations that are receiving funding from St. Josephs the Coordinator will monitor their performance on program objectives and report results to the Community Health department management. The Coordinator will also provide feedback and instruction to community based organizations to assist them in improving their program performance. The Coordinator must maintain appropriate documentation of services provided maintain budget parameters and complete required reports for St Josephs Medical Center. The position may require coordinating program activities from multiple grant and funding organizations. Administrative tasks include scheduling classes securing locations for health education events coordinating with other programs and agencies contacting patients and community members by phone and mail and tracking services provided along with outcomes The Coordinator will represent SJMC in meetings within the community and is responsible for submitting project data and outcomes. The Coordinator will also complete other assigned tasks and assist with additional community health programs based on department needs. This position requires the full understanding and active participation in fulfilling the Mission of the Organization. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support the Organizations strategic plan and the community benefit goals.

Qualifications

Experience:

* Minimum of one (1) year Community Outreach experience.
* Experience in addressing health disparities.
* Networking experience with community based organizations is desired.

Education:

* High school diploma or equivalent.
* Bachelor's Degree is preferred.
* Competency in writing skills, computer skills, and public speaking.

Special Skills:

* Cultural competency to provide effective outreach to one of the major undeserved populations of the area.
* Independent thinking, flexible and self-directed. Comfortable working in undeserved communities. Excellent verbal and written communication skills required.
* Bi-lingual in Spanish or one of the threshold languages of San Joaquin County preferred.
* Training related to public health issues and effective approaches for community health improvement.
* Knowledge of community needs and existing health resources.
* Familiar with statistics, quality improvement methodology, and adult learning principles a plus.
