Description

SUMMARY:

The Patient Care Navigator for DHH's refugee clinic will be instrumental in guiding new arrivals through the healthcare process. They will interface with government agencies, refugee resettlement agencies, clinicians, nurses, case managers, financial counselors, family counseling, pharmacy staff, and patients. For many of these patients, you will be their first contact with the US health system. It is therefore imperative to provide non-judgmental patient centered services to a diverse population of patients.

The role requires a resourceful, supportive, and non-judgmental individual who is comfortable working collaboratively with clinical, operational, patient services, marketing/communication, and project management staff at Dot House to drive positive health outcomes.

GENERAL DUTIES & RESPONSIBILITIES:

• Organizing referrals from government agencies and refugee resettlement agencies

• Contacting potential referrals for scheduling appointments and assessing needs for other services like case management or specialty medical services

• Relaying information back to government agencies needed to close the loop

• Coordinate with teams within DotHouse to address barriers to care, including navigating insurance options, medication assistance programs, addressing health-related social needs like transportation, and referring to additional services like behavioral health

• Link patients to primary care within DotHouse

• Serve as the liaison between the clinical care team and the patient

• Engage and empower patients with their own care, promoting self-navigation through the health care system

• Stay current with any relevant trainings related to refugee resettlement

• Document all patient care in EPIC in a timely manner, following established workflows and protocols

• Participate in quality improvement and reporting activities as requested.

Requirements

REQUIRED SKILLS & COMPETENCIES:

Education:

• High School diploma or GED

Experience:

• Strong demonstrated commitment to health equity required

• Independent and self-motivated

• Strong sense of professionalism, boundaries, and ethics

• Excellent interpersonal skills

• Bilingual Haitian Creole Required

• Able to collaborate effectively with diverse stakeholders including community members, patients, staff, and providers

Software/Hardware:

• Must be proficient and have a strong working knowledge of Microsoft Office (Outlook, Excel, Word, etc.)

Preferred:

• Associate degree preferred

• Bilingual Spanish highly desired

• Knowledge of Epic preferred
