POSITION SUMMARY: The physician is an essential member of a multidisciplinary team that provides health care to patients in an urban community health center serving a diverse community. Broadly, she/he will be a key player in developing and supporting the infrastructure to implement a preventive and chronic care model of team-based care delivery, with a patient-centered and continuous improvement perspective.

Our Health Center offers a range of primary and specialty health care for adults and children. We provide specialized clinics, Eye Care, Dental, Pediatrics, and OB/GYN.

MINIMUM QUALIFICATIONS:

A. Education:

Required - MD/DO, completion of an accredited residency in Internal Medicine or Family Medicine,

Board certified or eligible

B. Experience: required: residency as above, Bilingual Spanish preferred

C. Knowledge/Skills:
• Excellent oral and written communication skills.
• Strong organizational skills
• Great self-starter who integrates with multi-disciplinary team
• Motivational skills with patients
• Basic electronic documentation skills

D. License/Registration/Certification: Active Massachusetts licensure; DEA; Massachusetts controlled license. Board eligibility. Basic Life Support within 90 days of start.

PRINCIPAL ACCOUNTABILITIES (Job Specific Competencies):

JOB KNOWLEDGE - The extent to which the employee demonstrates technical and professional knowledge of job requirements and takes personal responsibility to maintain and improve that knowledge:

A. The employee is able to fulfill all of the requirements of the job assigned in a satisfactory manner as demonstrated by:

1. Clinical Care - Care for patients as part of the care team of physicians, mid-level providers, residents, nurses, medical assistants, social workers, nutritionists, and non-clinical administrative staff in each of the primary care clinics. In primary care departments, physicians will have his/her panel of patients for whom they and their care team are responsible. While physician is accountable for own treatment decisions regarding patient care, it is expected that the physician will work closely with the care team in devising both individual care plans for individual patients, as well as maintain a broader systems approach to addressing health of populations-his/her primary care panel as well as subgroups of patients with chronic conditions or otherwise identified patient populations at high risk.

2. Laboratory and Image Review - Review and sign off all laboratory and imaging results from patients. Work with team on systems for follow up of any abnormal result, communication of normal results, and management plan for no show labs/imaging.

3. Referrals/transition of care-- Review and sign off all transition of care documentation (ED, hospital, referrals) from patients. Work with team on systems for follow up of any outstanding needs, transitional needs, and management plan for no show referrals.

4. Access-accessible for clinical care both in person as per agreement on contract and after hour's phone consultation as per clinic specific rotation schedule.

5. Documentation-for purposes of seamless communication and data collection, PCP clearly documents all clinical interactions (in person and by phone) in the electronic health record in accordance with health center and clinic documentation workflows.

6. Patient Centeredness-as do all care team members ensures that the patients' needs are at the center of all treatment plans and broader clinical operations plans, and actively involves patients in development of such plans.

7. Community member/citizenship -.

a) Self-monitoring: ensures maintenance of own knowledge base. Ensure that own interpersonal skills with patients and other clinical staff is always professional and courteous.

b) Team monitoring: Work with team members to enhance their skills and plan for continuing training in clinical and customer service areas. Helps to highlight to team the big picture of how the department runs smoothly. Ensures that staff interpersonal skills with patients and providers are always professional and courteous. Ensures that team operations are systematic and efficient. Maintain communications with clinical director regarding performance of care team. Clear knowledge of process to communicate to appropriate administrators all performance issues for team members.

8. Continuous Quality Improvement - Accountable for individual and team quality measures. Participation in team or department discussions to optimize data documentation and to take data/measurement to design improvements to processes. Help to develop interventions in current systems/processes that to lead to improved outcomes for patient, populations, and clinic operations.

9. Participation in clinic specific projects-might include internal improvement projects, research projects, and grants.

QUALITY - The extent to which the employee performs assignments accurately and completely, and consistently meets the job requirements with minimal supervisory oversight and follow-up:

A. Adheres to the center/departmental policies, procedures, and regulations.

C. Assists in meeting scheduling needs in helping to cover for absences, emergencies, times of heavy workload, holidays, and vacations.
