Job Description

TOUR 2: 9:00 A.M - 5:00 P.M

AREAS OF RESPONSIBILITIES
  • Provides assistance to executives, leaders, and staff in a wide range of activities in the management of a hospital and/or medical center including such areas as hospital administration, total management and administrative systems, professional, medical, technical, operational, and related support activity services.
  • Assists in the administration management of specified activities and functions. Carries out assignments and undertakes studies involving the total service center or specific patient services, including budget planning, cost studies/analysis, and preparing specific budgets.
  • Collects and analyzes data needed as a basis for administrative and management decisions relative to various hospital activities.
  • Accumulate data for budget preparations, fiscal reports, and review of budget estimates.
  • Investigate and report on patient and visitor complaints and perform special studies and investigations on operational units.
  • Prepares and develops informational data in-hospital training and participates in the development and updating of activity programs.
  • Participate in Regulatory Surveys.
  • Liaison with NYSOMH and NYJC.

SUMMARY OF DUTIES AND RESPONSIBILITIES

VOICE: File

Responsible for review, follow-up and closure of VOICE Files which includes Incident/Risk Open Files

Review of Staff Input, and Good Catch Open Files, Review of VOICE Alerts

Assess, Run and Create Reports as Needed for BH/Psych

Review of cases for potential NYPORTS reportable incidents, and the submission of the NYPORTS report to the NYSDOH.

Initiates the investigation of BH/Psych Incident/Adverse Events through:

Chart Review

Staff Interviews-Staff with knowledge of the case

Review of video surveillance footage when appropriate and/or available.

Creation of timelines related to the incident/event and video surveillance footage timelines.

Review of Behavioral Health Incidents to determine reportability to NYS OMH and the NYJC.

Completion of NYS OMH and NYJC Special Investigation of Reportable Incidents, which include:

Video surveillance review, staff and patient interviews, EMR, other documents, policy review, and timelines

Coordinate NYJC Investigations with the NYJC Investigator and the NYJC Three Business Day Review Unit

Gather and submit requested information to the NYJC/NYSOMH

EMR, Assignment Sheets, Shift to Shift Reports, Level of Observation Reports, Staff Interviews Training Records, Video Surveillance Footage with Timelines, Photos of Injuries and any other documents requested,

Assist the NYJC/NYSOMH in obtaining Certified Copies of EMR as requested.

Coordinate, Interface, Communicate with the NYSOMH Clinical Risk Manager regarding reportable cases and or concerns.

Completion and submission of NYSOMH requested RCAs, submission of requested NYSOMH RCA Status Reports

Informs Chief of BH/Psych, CQO and CMO of reportable incidents

Request case summaries from appropriate staff.

Completes and emails to Central Office Risk Management: NOE, Initial NYPORTS Report, and VOICE Report related to incident/event.

Assist in maintaining the Risk Management BH/Psych Master Dashboard to ensure receipt of case reviews, updates, conclusions of reviews, submission of reports, including CAPs and MOEs.

Assist with BH/Psych Incident Review Committee (IRC) Meetings, prepares outlines of cases to be presented and discussed at IRC meetings.

Prepare draft of case summaries for presentation in Quarterly Board Reports and provides final cases presentations for the Quarterly Board Reports.

Assist with the review of policies and procedure and/or corrective actions designed to improve patient care and reduce risks.

Assist with the preparation of the monthly Quality & Patient Safety Committee (Q&PSC) reports, and the quarterly Quality Council Executive Committee (SCERC) Reports.

Ensures CAPs and MOEs are submitted to NYSOMH and the NYJC as requested, as related to outcomes of investigation, and Root Cause Analysis (RCA).

Completes collection and aggregation of data related to NYSDOH, CMS, JCAHO, NYSOMH and NYJC survey findings with Corrective Actions (CAPs) and Measures of Effectiveness (MOEs).

Participates in Board Report and Board Report prep meetings as assigned.

Participates in Joint Commission and other regulatory survey preparations and surveys.

Responds to daily inquiries from staff concerning BH/Psych Risk related issues and also serves as a resource person for safety issues.

Participates in departmental and hospital wide committees.

Take 24-7 call for Risk Management BH/Psych related issues.

Collaborate with Central Office Legal Affairs and Central office Risk Management as needed.

Other duties as assigned.

Minimum Qualifications
1. Master's degree from an accredited college or university in Hospital Administration, Health Care Administration, Public Health or a related discipline; and
2. Four (4) years of administrative experience and background at responsible management and administrative levels; or
3. An equivalent combination of training, educational background and experience in related fields and educational disciplines may be substituted on a year-for-year basis.
4. A valid and current Basic Life Support (BLS) Certification through the American Heart Association (AHA).
