Intensive Mobile Treatment (IMT) Registered Nurse

Reports to: Director, Intensive Mobile Treatment Team

Program Overview:

The Intensive Mobile Treatment Team is a mobile team that aims to improve program participant's quality of life through engagement in treatment, rehabilitation, housing, and support services that promote wellness, recovery, community integration, as well as community safety. The IMT team provides these services and/or, where appropriate, link consumers to these services. The Intensive Mobile Treatment Team is guided by the principles of access, engagement, continuity of care, and awareness of risk. Services include, but are not limited to assessment and treatment planning, crisis, intervention, medication management, medication assisted treatment, integrated mental health, substance use treatment, individual and group therapies, psycho-education, psychosocial an vocational rehabilitation, assistance with medical concerns and medication, linkage to follow -up assistance to primary medical care including on-going assistance with activities of daily living, entitlements, housing procurement and support, linkage and coordination with criminal justice system, shelter system, and assistance in using family and community services.

Position Overview:

The Registered Nurse (RN) is responsible for providing services within the scope of their nursing license to program participants on the Intensive Mobile Treatment team. The RN works closely with the psychiatric provider to coordinate behavioral health care and coordinates primary care with outside providers. The RN has a thorough understanding of what it means to approach clients from a person-centered perspective that incorporates Evidence Based Practices including Harm Reduction, Motivational Interviewing, Trauma Informed Care, and CBT.

Responsibilities
  • Improve the overall health of program participants, including promotion of physical and mental health integration.
  • Conduct health screenings, provide linkage to primary and specialty care, and educate on health and wellness.
  • Perform blood draws, monitor, and administer psychotropic injections consistent with consumer needs preferences and practice standards, and preparation of medication to be dispensed.
  • Complete progress notes and assessments in the electronic health record within a timely manner.
  • Work with external stakeholders - such as hospitals, shelters, jails, primary care - to achieve positive client outcomes.
  • Provide services in a strengths-based, recovery-oriented, trauma-informed, and culturally sensitive manner.

Qualifications, Skills, and Knowledge:
  • Active New York state license as a Registered Nurse
  • At least two years of experience working with adults who have needs in the areas of mental health, substance use, trauma, violence risk, homelessness, frequent psychiatric hospitalizations, and criminal-justice involvement.
  • Previous use of an electronic health record, Footholds AWARDS preferred.
  • Experience in a multidisciplinary setting and field-based clinical work preferred.
  • Understanding of the impact of the impact of systemic and historical racism on the lives of many of the program participants.
  • Excellent communication skills, both verbal and written.
  • Bilingual Spanish is highly desired.
  • A passion for the mission of a community based behavioral health program.
