Job Summary Responsible for following up with patients who have been registered for care in the emergency department with a focus on understanding of discharge instructions, identifying indications patient should return for re-evaluation and the degree of urgency, informing patient of test results, determining intent to pursue follow-up appointments as indicated, liaison between patient and provider to ensure appropriate medication therapy and/or need for further outpatient testing. Timely documentation in patient records as indicated. ED Call-Back nurse has a professional obligation to participate in committee activities, surveys and improvement initiatives. Job Requirements # Minimum Education Graduate of#an accredited school of nursing Minimum Work Experience 3 years in an acute care setting Required Licenses/Certifications Registered Nurse Required Skills, Knowledge, and Abilities Excellent human relation skills with ability to empathize, de-escalate, and actively listen Provide appropriate direction with corresponding urgency based on ability to glean adequate information Prioritize patient lists and collaborate with ED providers to ensure appropriate care loop closures Professional demeanor and phone etiquette Firm understanding of privacy regulations and how to establish acceptable recipient of information on a phone call General understanding of computer technology, electronic documentation, and Microsoft Office documents Autonomous with self-initiative, desire to serve patient needs and ability to work in a largely isolated environment #

Job Summary

Job Requirements
