Responsibilities:

* Provide high-quality, patient-centered care in the comfort of patients' homes
* Conduct comprehensive assessments and develop individualized care plans
* Administer medications, monitor vital signs, and manage patients' overall physical and mental well-being
* Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic care
* Educate patients and their families on disease management, treatments, and preventive measures
* Document and maintain accurate records of patients' progress and treatment plans

Qualifications:

* Active and unrestricted Registered Nurse (RN) license in the state of Florida
* Minimum of 1 year of nursing experience, preferably in home health or a related field
* Strong clinical skills and ability to provide comprehensive care to patients of all ages
* Excellent communication and interpersonal skills
* Ability to work independently and make sound clinical judgments
* Proficiency in electronic medical record (EMR) systems
* Valid driver's license and reliable transportation to travel to patients' homes
