As the Hospice Spiritual Care Coordinator you would be responsible for assessing and addressing the spiritual/pastoral, bereavement and additional individual and/or group counseling needs. Develop and implement individualized spiritual plans of care in collaboration with local clergy and the interdisciplinary team. Conduct religious ceremonies as appropriate.

PRN - Various Hours As Needed

MAJOR ACCOUNTABILITIES/ESSENTIAL JOB DUTIES:

* Be available to the patient/family, hospice staff and volunteers, as needed, to assess and address the spiritual/ pastoral, bereavement and/or the additional individual and/or group counseling needs
* Provide spiritual/pastoral counseling services in accordance with the plan of care.
* Complete a spiritual/pastoral assessment as indicated and appropriate as soon as possible after admission.
* Provide spiritual/pastoral support in accordance with on-going wishes and needs of the patient/ family.
* Develop an individualized spiritual/pastoral plan of care, which demonstrates an effort to work in close collaboration with local clergy and WCR Pastoral Care staff whenever desired by the patient; provides spiritual support as needed and defined by the patient/family
* Conduct religious services of prayer, worship and rituals for patients/families as appropriate
* Offer patients/families of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feelings, beliefs, and values
* Document counseling provided in the patient's clinical record
* Work with other professionals in resolving spiritual/pastoral/ethical issues
* Work in a team approach with the IDT/IDG and other qualified professionals, as determined by the hospice, to evaluate patient / family response to care providing input to the Plan of Care & participate in the delivery of that care.
* Attend IDT/IDG conference when plan of care is reviewed.
* Upon request, meet with individual staff regarding personal spiritual/pastoral issues that may affect their ability to function effectively
* Assist in developing and updating the plan of care
* Assess patient/family response to and satisfaction with care
* Participate in development of approaches to meet staff counseling needs
* Make visits and/phone calls to the patient/family prior to death and is available to family for bereavement support upon request and direction of the Administrator
* Be available and provide bereavement support to staff
* Participate in Westminster Canterbury Richmond Pastoral Care staff meetings to coordinate care
* Must be available after hours/weekends/holidays to meet the needs of patients and their families when requested
* Additional duties as assigned

REQUIRED KNOWLEDGE, SKILLS AND ABILITY:

* Basic computer skills
* Must possess excellent verbal and written communication skills
* Must be able to meet deadlines, work under limited supervision.
* Ability to compassionately and patiently work in high stress situations including end of life and hospice issues
* Provide pastoral leadership, actively listen, and assess and respond the spiritual needs of others with a non-anxious presence
* Ability to handle interruptions throughout the day and be able to prioritize workload
* Strong interpersonal skills, including relationship building with staff, residents, families, and the outside religious community
* Strong personal faith with the ability to be able to deeply connect with and care for those of differing faiths
* Valid Driver's License in the State of Virginia with a minimum age 21 to drive for the organization
* Motor Vehicle Driving Record satisfactory to the Agency and its insurance providers
* Proof of motor vehicle insurance

DESIRED KNOWLEDGE, SKILLS AND ABILITY:

* Theologically sound and inspiring speaker
* Strong sense of intuition when making pastoral visits, ability to help others more fully examine spiritual issues
* Strong knowledge base of community clergy

REQUIRED EDUCATION AND EXPERIENCE:

* Experience in providing end of life spiritual care for patients and families with multiple faiths and cultural perspectives
* Knowledge of local faith community resources

DESIRED EDUCATION AND EXPERIENCE:

* Ordained, commissioned or credentialed according to the practices of an organized religious group
* Experience in home health care or hospice
* Experience in working with geriatric or cognitively impaired residents
