As an Clinical Liaison at Georgia Hospice Care, you’ll be responsible for coordinating & generating hospice business in both existing and new accounts, identifying new markets and maintaining existing service as well as coordinating care for referred patients. The Clinical Liaison's primary responsibility is serving as a liaison between the agency, hospitals, medical community and other referral sources. evaluate patients and create care plans, all while communicating with everyone involved — the patient, the patient’s family, and the care team. You’ll serve as the driver of our care team to ensure every patient receives quality care.

Qualifications:
  • A heart to serve patients and families and a passion for providing the best possible care
  • Education: Registered Nurse currently licensed in the state. Must successfully perform and comply with all credentialing as required by hospital/healthcare facility. 2 year experience in a clinical care setting or hospice preferred.
  • Experience 2 years of sales experience in a clinical care setting, hospice preferred.
  • Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
