Full Job Description
Primary Duties and Responsibilities
The Home Health- Nurse Practitioner is responsible for assisting in the expedient multidisciplinary care of patients for Hoag Hospital and its affiliates, coordinating seamless patient care from admission through discharge in the Home Health setting. Providing supplemental medical care to patients with a focus on quality and patient experience, the Nurse Practitioner can perform history and physical exams, diagnoses and treats medical problems, orders and interprets diagnostic and laboratory tests, performs minor procedures and facilities the discharge process.

Job Knowledge – Demonstrates full knowledge and understanding of concepts and procedures needed to be successful in the role, including knowledge of applicable department, organization and external rules and regulations impacting day to day processes. For clinical positions, this includes clinical procedures required by department and unit, as demonstrated by unit-based competency testing.
Oral/Written Communication - Ability to speak and write effectively, demonstrates fundamental command of language, communicates well using all mediums.
Patient Documentation – Prepares and delivers patient information in a manner that is consistent with the requirements of the role. Includes what happens during a clinical visit with patients, why a procedure or service is being performed and/or why a particular device is being used.
Patient Safety – Embraces the principle of keeping patients safe by following safety policies and procedures, treating people with respect, and speaking up in Patient Safety related situations.
Travel – Position may require local travel between various Organization locations

Education and Experience
Required:
Master’s degree in Nursing
At least 1-2 years of clinical NP experience
Advanced clinical knowledge and skills in the care and management of patients
Solid computer technical skills required, including proficiency with Microsoft Office products (Outlook, Word, Excel, PowerPoint, and Visio)
Preferred:
Two years recent clinical experience as a Nurse Practitioner, preferably within an ambulatory and acute healthcare environment, related to the care of Orthopedic patients.
Qualified bilingual or multilingual candidates
Licenses Required
Current license in good standing to practice as a Registered Nurse/Nurse Practitioner in California
Furnishing license issued by the State of California and personal DEA number.
Certifications Required
BLS and ACLS certification
National board certification as Nurse Practitioner in area of specialty
