Job Description Metro is looking for a motivated and passionate PER DIEM Occupational Therapist to provide in-home care. Job Type: Per Diem Pay: $75-$90 per visit (can choose either W2 or 1099) Location: In and around Forest Hills Schedule: Flexible What We Offer 1099 and W-2 options to fit your preferred work style Competitive salary Longstanding Success Opportunities for professional growth and development A professional, supportive and fun work environment fostered by a team of well-respected clinicians who take pride in the challenges of meaningful work Focus on Patient Care - Quick EMR (No Oasis required) Great work/life balance Flexibility and Autonomy - see patients any 7 days of the week Preference given to see patients in your primary geographic area Responsibilities: Accept and treat patients within your designated geographic area Provide 1:1 treatment Evaluate patients and develop individualized treatment plans based on physician orders and patient needs Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care Educate family members, home health aides, and other caregivers on proper therapy techniques Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients. Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention. Provide timely and accurate documentation for each visit Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit Must be willing to treat geriatric patients. Qualifications Occupational Therapist Program Graduate Licensed Occupational Therapist in the state of New York or eligible. Mandatory Certifications: First Aid, AED, & CPR Certifications. Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance. Ability to travel to patients' homes within the assigned geographic territory Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc. Additional Information We are proud to be an equal opportunity and affirmative action employer.
