*JOB DUTIES*
* Assess home health pediatric patient for rehabilitative and functional status.
* Regularly re-evaluate the patient’s/client’s speech and language need and evaluate the outcomes of care on a regular basis.
* Speech Language Pathologist develops, initiates, and revises the plan of care as necessary to ensure quality and continuity of care.
* Identifies deficits including but not limited to communication and feeding / swallowing and potential for intervention. Evaluates the home environment.
* Speech Language Pathologist evaluates the need for and recommends communication devices and aides.
* Uses infection control measures that protect both the staff and the patient (OSHA).
* Coordinates services. Informs the physician and other personnel of changes in the patient‘s/client’s condition and needs.
* Speech Language Pathologist demonstrates commitment and professional growth by participating in in-service programs and maintaining or improving competency.
* Supervises, teaches, and provides clinical direction to other speech and language personnel as required or requested.
* Promote Agency philosophy and administrative policies to ensure quality of care.
* Speech Language Pathologist provides effective communication to patients/clients, their family members, team members, and other health care professionals.
*QUALIFICATIONS*
* Graduate of an accredited Diploma, with Bachelor’s, Master’s, or Doctorate degree
* One year of experience as a Speech and Language Pathologist in a pediatric clinical care setting preferred
* Pediatric Home health experience preferred
* Reliable transportation
* Current Texas State license as a Speech Language Pathologist
* Current Driver’s License
