Job Description:

We are seeking a dedicated and compassionate Physical Therapist to join our team in Sierra Vista, AZ, on a full-time basis. As a Home Health Physical Therapist, you will have the opportunity to make a meaningful difference in the lives of our patients by providing high-quality physical therapy services in the comfort of their homes.

** Up to a $10,000 BONUS**


Responsibilities:

  • Conduct comprehensive assessments of patients' physical abilities and needs
  • Develop individualized treatment plans to address patients' functional limitations and promote independence
  • Provide skilled physical therapy services, including therapeutic exercises, manual therapy techniques, and education on self-management strategies
  • Collaborate with other members of the healthcare team, such as nurses, occupational therapists, and speech therapists, to ensure coordinated and holistic care
  • Monitor patients' progress and adjust treatment plans as needed to achieve optimal outcomes
  • Document all patient care activities accurately and timely in accordance with company policies and regulatory requirements

Qualifications:

  • Valid Physical Therapy license in the state of Arizona
  • Minimum of 1 year of experience in physical therapy, preferably in a home health or a related setting
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Reliable transportation and willingness to travel to patients' homes within Sierra Vista and surrounding areas
