Job Summary

Under the supervision and direction of the Director of Nursing and Primary Care Clinic, the Clinic RN Manager is responsible for the daily operations of the primary care clinic and the nursing department. Supervises, directs and coordinates clinical staff functions. Responsible for ensuring the clinical staff operates efficiently and effectively.


Essential Job Functions

Duties included, but not limited to:

Manage and supervise daily clinic operations and nursing department; staffing, scheduling, nursing staff supervision and quality oversight.

Responsible for the supervision of the clinical staff including Registered Nurses, Licensed Vocational Nurses, Medical Assistants Phlebotomists, Clinic Schedulers and Certified Nursing Assistants, which are based on the needs of each site.

Training new employees, competency assessments, training development, coaching/counseling, evaluating performance and preparing written performance appraisals; listening to complaints and effectively resolving disputes or problems; taking

corrective or disciplinary action; developing work schedules for subordinates; and approving leave requests.

Supervises Clinic schedulers to ensure timely completion scheduling initials, reassessments, follow up and laboratory appointments.

Participates as a member of the interdisciplinary team (IDT), including participation in care coordination meeting for new enrollees.

Responsible for monitoring, reviewing and reporting of COVID19 testing and COVID-19 vaccination and other vaccination/immunization administered to recipients. Ensuring all necessary documentations are completed and accurate.

Responsible for the implementation, monitoring and maintenance of all clinic logs, including monitoring, administering, and recording ordered medication.

Oversees clinic supplies, expiry dates, and ensures clinic equipment is in operating condition including the isolation cart, emergency cart.

Monitors clinic equipment maintenance and contacting outside vendors for upkeep and repairs.

Coordinates all medication for IIH PACE, including the stocking of non-prescription medications, the ordering of prescription medications, the administering of medication in the PACE Center, and the monitoring of participant compliance. Arranges for delivery of needed medications and supplies to the participant’s home.

Involved in the development and implementation of Quality Improvement activities; evaluates overall effectiveness of the center, implementing change and quality improvement, as needed.

Provide for adequate staffing at the facility; fill in as needed during periods of short staffing

May perform nursing duties including, executing physician’s orders, assisting physicians in examinations or treatment, dispensing and administering medications, treating emergencies and screening patients for referrals, and performing intakes.

Perform triage and physical assessments.

Assists with preparing for California Department of Health Care Services (DHCS) and Centers for Medicare and Medicaid Services (CMS) audits.

Assist in monitoring and completion of Quality Improvement reports; Precaution list, QI initiatives and matrix, Grievances, Incident reports and Med A reports.

Monitors and assist in communicating with providers any IIH Pharmacy correspondence.

Assists the Director of Nursing and Medical Director in hiring clinic staff.

Communicates with weekend and after-hours on-call staff, following up on issues, as necessary.

Reviews participant medical records to ensure timely and accurate and timely clinic staff documentation.

Monitors nursing assessments and care plan’s timely completions.

Monitor staff productivities with all clinic documentations and work related tasks and responsibilities.

Ensures both staff and own personal and professional development through attendance of in-services, meetings and continuing education courses

Acts as liaison with PCP in the event of an episodic illness; assists in coordinating services provided by PCP.

Collaborate with Director of Nursing on any new and current project improvements for the department and IIH as a whole.

Maintains confidentiality of participant information

Attend and participate in staff meetings, in-services, projects, and committees as assigned.

Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance.

Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.

Be flexible in schedule of hours worked based on facility needs.

May require use of personal vehicle

Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead; squatting and kneeling.

Some undesirable conditions at center may include exposure to odors, fumes, infections, dust, and dirt, which may be objectionable.

Some travelling requirement to another IIH site for coverage as necessary.

Local car travel frequently necessary; out-of-town travel is minimal.


Requirements

Experience

Minimum of three (3) years of health care experience with emphasis in geriatrics strongly preferred.

Minimum of one (1) year of documented experience in Primary Care Clinic or similar setting.


Knowledge, Skills, and Abilities

Knowledge of primary care clinic daily operations

Broad knowledge base of physical, mental, and social needs of the frail elderly population.

Knowledge of clinic medical equipment and instruments.

Knowledge of common safety hazards and precautions to establish a safe work environment.

Possess management and leadership skills and be able to manage staff greater than 10.

Experienced in physical assessment and triaging.

Skilled in multi-tasking, identifying problems and recommending solutions.

Able to effectively prepare and maintain records, write reports, and respond to correspondence in a timely manner.

Experienced in development and maintenance of department quality assurance.

Ability to react calmly and effectively in emergency situations.

Able to establish and maintain effective working relationships with participants, medical staff, staff members, and family caregivers in a pleasant, patient, and professional manner.

Well organized, dependable, flexible, and resourceful.

Effective oral and written communication skills.

Computer skills required.


Education and Certification

Completion of an Accredited Nursing program

Bachelors of Science in Nursing preferred

Current California Registered Nurse License

BLS certification

Is medically cleared for communicable diseases and has all immunizations up-to-date before engaging in direct participant contact.
