We invite you to become part of our dedicated team In-person floater role supporting MDS/NAC needs across multiple facilities with local travel only; no overnight stays, with mileage reimbursement. What You'll Do: As an MDS Nurse Assessment Coordinator, you will take charge of completing and ensuring the accuracy of Minimum Data Set (MDS) assessments for all residents. You will play a crucial role in developing personalized resident care plans and ensuring proper clinical reimbursement for services provided. Key Responsibilities: Assess prospective residents’ nursing needs and determine appropriate clinical reimbursement levels under the Patient Driven Payment Method (PDPM). Ensure the accuracy of the MDS process for all residents. Monitor Case Mix Index (CMI) scores and identify potential risks that may affect reimbursement. Maintain compliance with state and federal regulations for MDS completion and coding conventions. Collaborate with interdisciplinary teams to collect accurate data for assessments. Educate facility staff and leaders to enhance ongoing understanding and practices. The chance to make a meaningful impact in the lives of our residents. What You'll Bring: Qualifications for an MDS Nurse Assessment Coordinator include: A valid state RN nursing license. Advanced degree or certification preferred. Previous experience in a long-term care setting, MDS Coordinator, Clinical Reimbursement Specialist, or Nurse Assessment Coordinator preferred. Familiarity with state and federal regulations regarding the MDS, Electronic Medical Record (EMR), PDP, MDS 3.0, Medicaid, and Medicare requirements. A genuine interest in the nursing needs of the elderly and chronically ill. Strong organizational skills, analytical abilities, and the capacity to make independent decisions. Excellent written and verbal communication, with strong interpersonal skills. Ability to work effectively and influence others within a multidisciplinary team.
