Job DetailsJob LocationCarriage Court of Grove City - Grove City, OHSalary Range$28.85 - $30.28 HourlyDescriptionResponsibilities:Designs and facilitates the Community Memory Care program, promoting active participation from residents, family members, and team membersOversees the budget and implementation of a cost-effective Memory Care program, including managing an inventory list of equipment and supplies and maximizing resources availableCreates and sustains a welcoming and safe environment which promotes resident wellness and independenceParticipates in the resident move-in process through initial wellness assessments and the development of individualized care plansApproximately 20% of hours will be providinghands-on nursing responsibility and direct careTrains, schedules, and leads the Memory Care clinical team in providing exceptional care experiences, providing hands-on support, education and training opportunitiesDelivers health promotion services and dementia care education services for residents and familiesEngages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experienceContributes toward quality assurance and performance improvement initiativesCoordinates the Community's infection control program while maintaining state regulatory complianceQualificationsQualifications:Maintains current state Registered Nurse (RN) licensure, Licensed Practical Nurse (LPN),Social Work licensure, or Dementia Care Professional certificationBachelor's degree preferredMinimum of three (3) years experience in dementia care. memory care/assisted living experience preferred.Must possess leadership and supervisory ability and the readiness to mentor, motivate, and inspire team membersExcellent written and verbal communication skillsDemonstrates exceptional interpersonal skills and ability to serve with love, compassion, and dignityBenefits:Unlimited growth opportunitiesMedical, dental, and vision benefit packages availableWork-life balance | Paid Time OffEAP & Maven family planning program401(k) employer match biweeklySelf-service payrollDaily pay options available
