Role Description

This is a full-time role for a Home Health RN Clinical Team Leader at Holy Family Home Health and Hospice located in Parma, OH. This management position will oversee a team of healthcare professionals, coordinate patient care, ensure compliance with regulations, and provide clinical guidance and support.


Qualifications

  • Active Registered Nurse (RN) license
  • Prior experience in home health is required
  • Strong leadership and communication skills
  • Knowledge of Medicare and Medicaid regulations
  • Ability to collaborate effectively with interdisciplinary teams
  • Excellent problem-solving and critical thinking abilities
  • Demonstrated clinical expertise and proficiency in patient assessment
