Shift: Night 4x9-Hour
Pay Range: $1,500 - $2,000 Weekly Gross
Position Summary:
- As an Inpatient Social Worker with a Licensed Master Social Work (LMSW) designation, you will play a vital role in providing psychosocial support and advocacy for patients and families admitted to our healthcare facility.
- Working collaboratively with interdisciplinary teams, you will assess patient needs, develop care plans, and connect individuals with essential resources to facilitate their well-being and recovery.
Essential Functions:
- Conduct comprehensive psychosocial assessments of patients and families to identify their strengths, needs, and resources.
- Provide crisis intervention, emotional support, and counseling to patients and families facing medical, emotional, or social challenges during hospitalization.
- Collaborate with healthcare professionals, including physicians, nurses, and therapists, to develop and implement individualized care plans that address the social and emotional aspects of patient care.
- Advocate for patients and families to ensure their voices are heard in decision-making processes related to healthcare, discharge planning, and community resources.
- Facilitate communication and coordination between patients, families, healthcare providers, and community agencies to promote continuity of care and seamless transitions.
- Assess and address barriers to discharge, including housing instability, financial concerns, transportation issues, and access to healthcare services.
- Provide education and guidance to patients and families regarding available community resources, support services, and entitlement programs.
Education:
- Graduation from an accredited school of social work with a master’s degree.
Certificate/License:
- Current Licensed Master Social Work.
- WA state license required.
