*Qualifications:*
* Valid Physical Therapist license in the state of Colorado.
* Compassionate and patient-centered approach to care.
* Strong communication and interpersonal skills.
* Ability to work independently and as part of a team.
*Responsibilities:*
* Provide exceptional home health physical therapy services to patients in the Commerce City and Brighton areas.
* Develop and implement personalized treatment plans based on patient needs and goals.
* Collaborate with other healthcare professionals to ensure comprehensive patient care.
* Maintain accurate and up-to-date patient records.
Job Types: Full-time, Part-time, PRN
Pay: $70.00 - $75.00 per hour
