This role focuses on providing expert medication management and personalized care for complex patients who typically take 8-15 medications daily. The ideal candidate will possess strong retail and/or specialty pharmacy experience and have excellent communication and consultation skills, especially when working with patients managing chronic diseases such as diabetes, hypertension, and multiple sclerosis. Responsibilities include accurately dispensing medications, counseling patients on their complex medication regimens, and collaborating with healthcare providers to optimize therapeutic outcomes. Candidates should demonstrate patience and compassion, given the patient population primarily includes Medicare/Medicaid low-income individuals with varying literacy levels. This position emphasizes quality patient care over volume, requiring a pharmacist who values detailed consultation and disease state management.
