If you're a licensed PTA who is passionate about patient-centered care and values working in a supportive, collaborative environmentthis is the place for youPosition Summary: The Physical Therapist Assistant provides physical therapy treatments under the direction and supervision of a licensed Physical Therapist. This role supports residents in achieving their mobility and recovery goals as part of an interdisciplinary care team.Key ResponsibilitiesDeliver therapy services in accordance with the treatment plan established by the Physical TherapistMonitor patient progress and communicate findings to the supervising therapistAccurately document daily treatments, patient responses, and progress using systems such as Casamba and PointClickCareAssist with discharge planning and family education regarding home exercises or mobility assistanceEnsure compliance with all federal, state, and facility guidelines and policiesCollaborate with nursing, therapy, and administrative teams to support quality resident careQualificationsAssociate's degree from an accredited PTA programCurrent PTA license in good standing in the State of CaliforniaSkilled nursing or post-acute care experience preferredStrong communication, teamwork, and organizational skillsProficient with electronic medical records and therapy documentation platformsWhat We OfferCompetitive pay starting at $42/hourComprehensive benefits package and 401(k) for full-time staffSupportive work environment and team cultureCareer development and continuing education opportunitiesEmployee recognition and appreciation programsMake a difference every day at Linwood Meadows Care Center.
