Job Description Job Description Immediate need for home care private duty RNs, LVNs, CNA/CHHAs to work flexible schedule in the home from 4-24 hours/day. We also need RNS and LVNs for intermittent visits IMMEDIATE OPENING FOR FULL TIME LVNS - health plan & 401 K RN 8 & 12 hour shifts in Orange County & surrounding areas RN & LVN & CNAs & Caregivers for Orange County, CA to work 8 &8 & 12 hour shifts Private Duty shift care available immediately - 8 & 12 hour shifts Full time AND part time positions in all of Los Angeles and Orange County LVNs ongoing 8 hour shifts & 12 hour shifts 7AM to 7PM and 7PM to 7AM Intermittent Visits & 4 hour infusions for good infusion nurses CHHA - 4- 12 hour shifts (full time cases available) IMMEDIATE OPENING 12 HOUR SHIFTS FOR CNA IN NEWPORT BEACH LVNs - shifts and visits CALL 818-753-5106 AND SPEAK WITH HR OR PATI TODAY RATES RN $50 - $75/hour LVN $29 - $40/hour CNA $21 - $25/hour Company Description Continuity Care Home Nurses is a licensed, insured and bonded state-of-the-art home health agency consisting of a multi-disciplinary team of health care professionals that are dedicated to the patient care philosophy of "continuity of care." Our personalized service, knowledgeable staff, and close monitoring of services provided create a warm, supportive, and caring environment for patients as well as family members. We provide Certified Nurse's Aides & skilled nursing care (RNs, LVNs) from four hours/day to 24 hours/day. Our second company Continuity Care Staffing Services, Inc. provides babysitting & caregivers to pediatric and geriatric patients. We specialize in high tech home infusion visits & care to catastrophic patients (Ventilator Dependent, Spinal Cord Injury, Brain Injured) in addition to having PT, OT, ST, LCSW available. All cases are supervised by Registered Nurses who also case manage each patient. We can bill all private insurance plans, worker's compensation, long term care insurance, & accept all major credit cards. Our service area includes most of Southern CA.
