Primary Care Physician Assistant Job Description: Primary Care Nurse PractitionerLocation: Gales Ferry, ConnecticutPractice: Gales Ferry Medical GroupSchedule:Work Week: 4 days (Monday - Friday) Flexible Options: Full-time (4 days per week) or Part-time (minimum 3 days per week) Position Overview: Gales Ferry Medical Group, a trusted independent medical practice serving southeastern Connecticut since 1970, is seeking a Nurse Practitioner (Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner) to join our team. The role involves providing comprehensive primary care services, including sick and well visits and some gynecologic care, to a diverse patient population. Key Responsibilities:Setting: Outpatient only Patient Volume: 18-22 patients per day Conduct comprehensive patient assessments, including routine sick and well visits. Provide gynecologic care as part of primary care services. Diagnose and manage acute and chronic conditions with evidence-based treatment plans. Promote preventive care and health maintenance strategies tailored to patient needs. Collaborate with physicians and other healthcare professionals to ensure high-quality, coordinated care. Maintain accurate and timely documentation in compliance with healthcare standards. Qualifications:Certification: Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) Active Nurse Practitioner license in Connecticut (or eligibility to obtain one). Minimum of 1-2 years of primary care or outpatient experience required. Strong communication skills and a commitment to patient-centered care.
