Arrange and coordinate appropriate health services, including urgent, emergent and preventative health services, for children in care. This includes maintaining complete, confidential medical records for children, ensuring appropriate communication of health needs to case managers and clinicians, scheduling health care appointments, coordinating transportation to health care appointments, ensuring qualified interpretation and translation services are in place, and serving as a liaison for community health care providers.
