Palliative Care Coordinator Qualifications:

  • Must possess a current, unencumbered LPN License
  • Experience in scheduling in a healthcare organization or unit
  • Receive and promptly process palliative care referrals and/or inquiries for services provided by Crossroads
  • Work in cooperation with the call center for incoming or received referrals
  • Adept knowledge of Palliative Care program and ability to explain to the fullest extent palliative care utilization and services available
  • Maintain accurate and detailed records of all information calls and referrals
  • Schedule and assign as appropriate all palliative patients with designated Palliative Care Nurse Practitioner(s)
  • Additional duties as assigned
