A home care organization in Washington is seeking a licensed Nurse Practitioner (NP) and/or Physician Assistant (PA) to join their team to provide support in the Puyallup area.

About the Opportunity:

  • Schedule: Monday to Friday
  • Hours: 8am to 5pm
  • Patient Population: Geriatrics
  • Patient Volume: 6 to 8 per day
  • EMR: ECW

Responsibilities:

The Primary Care Provider (NP / PA) will:

  • Provide compassionate, holistic patient care, addressing diverse needs and collaborating with multidisciplinary teams
  • Deliver exceptional clinical care in-person and virtually, with in-person settings including private homes, congregate living facilities, and/or other home settings
  • Integrate practices and processes to achieve company goals
  • Maintain accurate medical records and participate in initiatives for patient engagement
  • Collaborate with external providers and partners
  • Perform other duties, as needed

Qualifications:

  • Graduation from an accredited Health Professionals school
  • NYS Nurse Practitioner (NP) and/or Physician Assistant (PA) license
  • Successful completion of national Board Certification
  • Active DEA licensure and state-specific Controlled Substance registration, as applicable.
  • BLS certification
  • Clinical experience caring for aging Adults and Adults with complex care needs
