As a Physical Therapist with Banner Home Health, you will work within designated geographic areas, ensuring shorter travel distances compared to many other agencies. You will benefit from an independent yet supportive work atmosphere, with options for pay per visit or hourly compensation. We provide mileage reimbursement and equip you with a company cell phone and tablet for documentation in our electronic medical record system.

In this role, you will have the opportunity to treat patients with diverse clinical conditions and objectives, including post-operative orthopedic cases, neurological rehabilitation, cardiac issues, and more. This position entails home visits primarily on weekends (Saturdays and Sundays), with flexible scheduling options for additional workdays. We are considering both full-time and part-time candidates.

POSITION SUMMARY
This role involves evaluating, planning, administering, and supervising Physical Therapy treatments in accordance with physician referrals, state and national standards, regulatory guidelines, and established departmental policies and procedures. The position requires clinical competence and knowledge to provide age-appropriate treatment for the patients served.

CORE FUNCTIONS
1. Assess the patient's functional abilities and limitations, determining the need for intervention. Develop a comprehensive plan of care and treatment goals within the established timeframe. Collaborate with patients and their families when setting goals.

2. Implement a tailored treatment plan considering the patient's physical, social, spiritual, cultural, educational, and age-specific needs. Provide skilled treatment safely and assess ongoing effectiveness, modifying the plan as necessary. Involve patients and families in the treatment process.

3. Maintain documentation according to professional practice guidelines, ensuring legibility and timely completion. Document evidence-based treatment techniques and patient progress.

4. Communicate relevant patient information to ensure continuity of care, sharing evaluation and treatment results with the healthcare team in a clear and timely manner.

5. Demonstrate flexibility in assignments related to caseload and scheduling, assisting in other patient care areas as needed.

6. Supervise and delegate tasks to ensure safe, quality patient care, participating in the training and orientation of new personnel.

7. Engage in departmental operational activities, attending required meetings and contributing to process improvement initiatives.

8. Interact with various staff levels, physicians, patients, families, and external contacts regarding patient care, ensuring effective communication and collaboration.

MINIMUM QUALIFICATIONS
Must have graduated from an APTA accredited Physical Therapy program or equivalent. A current Physical Therapy license in the state of practice is required, along with BLS certification. Candidates should demonstrate clinical competency and relevant experience throughout the interview process.

PREFERRED QUALIFICATIONS
Additional related education and/or experience is preferred.
