Enrollment/UAS Assessment Nurse Responsibilities Conducting pre-enrollment assessments for prospective enrollees utilizing clinical nursing expertise and critical thinking skills including obtaining preventive health service data.  Completes UAS - New York Assessment instrument to determine continuing eligibility and / or identify any needed changes to Care Plan. Utilizes a skillful approach for assessing, coordinating, and evaluating consumer needs and will need to develop a prospective plan of care.   Communicates with the Care Team and participates in interdisciplinary team meetings.
