Responsibilities As a Nurse Practitioner, you will conduct annual risk assessments in the home setting or via telehealth, operating within your scope of practice as directed by the Collaborative Physician. Essential Duties and Responsibilities Perform comprehensive in-home health risk assessments, identifying active and chronic conditions along with physical, mental, and social needs. Take patient history, perform examinations, and establish diagnoses. Document patient visits according to NCQA standards. Measure patient vital signs as needed. Facilitate case management referrals and collaborate with PCPs as necessary. Engage with patients, caregivers, agency nurses, and other healthcare providers to ensure accurate diagnoses. Adhere to OSHA Universal Precautions in all clinical duties. Maintain patient confidentiality at all times. Attend mandatory meetings and participate in professional development activities. Continuously uphold professional licenses and affiliations. In this role, you will collaborate with: Teammates Physicians Medical Staff Patients Caregivers Agency Nurses Providers Vendors Qualifications Required Knowledge, Skills, and Experience Active and unrestricted nurse practitioner license to practice in coverage states. Board certification from ANCC, AANP, or NCCPA. Current CPR Certification. Active enrollment in Medicare/Medicaid. Valid driver's license with a clean driving record. Excellent electronic health record (EHR) skills. Preferred Knowledge, Skills, and Experience Experience in geriatric care. Strong teamwork skills to foster effective working relationships. Conditions of the Role: Ability to adapt to varying weather conditions and diverse home environments. Full range of body motion, including handling and lifting patients, manual dexterity, normal vision and hearing, standing, bending, and stair climbing. Regularly lift/carry items weighing up to 50 pounds. Drive or travel up to 150 miles daily in urban and/or rural settings, as necessary. Compensation Minimum Pay Rate: $85.00/Visit Maximum Pay Rate: $145.00/Visit Notice HarmonyCares and HarmonyCares Hospice are not affiliated with Harmony Hospice Care.
