The Physical Therapist (PT) is responsible for assessing, planning, and providing rehabilitation services to patients or residents in a healthcare, long-term care, or assisted living facility. The PT helps patients restore or improve physical function, mobility, and quality of life by developing personalized treatment plans focused on rehabilitation, pain management, and injury prevention. This role involves working closely with patients, their families, and the interdisciplinary care team to ensure the highest standard of care.

Key Responsibilities:

Patient Assessment:

Conduct comprehensive evaluations of patients physical abilities, functional limitations, and mobility needs.
Develop individualized treatment plans based on medical diagnoses, assessments, and patients‘ personal goals.
Collaborate with physicians, nurses, and other healthcare professionals to ensure a holistic approach to patient care.

Treatment Planning and Implementation:

Design and implement personalized therapy programs that address patients physical impairments, promote mobility, reduce pain, and prevent further injury.
Use a variety of techniques such as exercise, manual therapy, therapeutic modalities, and patient education to improve movement and function.
Adjust treatment plans as needed to maximize patient progress and recovery, based on ongoing assessments and feedback.

Therapeutic Interventions:

Provide therapeutic interventions aimed at improving strength, coordination, balance, flexibility, and range of motion.
Teach patients exercises and techniques to improve mobility and prevent falls or further injury.
Use assistive devices (e.g., walkers, canes, braces) to help patients regain independence in daily activities.

Patient and Family Education:

Educate patients and their families on treatment plans, home exercise programs, and strategies to maintain progress outside of therapy sessions.
Provide guidance on injury prevention and management of chronic conditions, such as arthritis or back pain.
Ensure that patients and caregivers understand the use of any assistive devices or equipment needed for rehabilitation.

Documentation and Compliance:

Maintain accurate and up-to-date documentation of patient evaluations, treatment plans, progress notes, and outcomes in the electronic health record (EHR) system.
Ensure compliance with federal, state, and facility regulations regarding physical therapy services and patient care.
Participate in care plan meetings, interdisciplinary team discussions, and quality improvement initiatives.

Collaboration and Communication:

Work closely with other healthcare professionals, including occupational therapists, speech therapists, and nursing staff, to provide comprehensive care.
Participate in discharge planning, offering recommendations for continued physical therapy services or home care as needed.
Communicate effectively with patients, families, and the care team to ensure that treatment goals are aligned with patient needs and preferences.
Education: Doctorate or Masters degree in Physical Therapy from an accredited program.
Licensure: Current and active Physical Therapist license.
Experience: Previous experience in a healthcare, long-term care, or rehabilitation setting is preferred but not required. New graduates are welcome to apply.
Skills:
Strong clinical and patient care skills with the ability to develop and implement effective treatment plans.
Excellent communication and interpersonal skills, with a compassionate and patient-centered approach to care.
Ability to work effectively as part of an interdisciplinary team and collaborate with other healthcare professionals.
Proficiency in electronic health records (EHR) systems and therapy documentation.
Knowledge of therapeutic techniques, assistive devices, and equipment used in rehabilitation.
