Rise Up Health Clinics is a nurse-led, non-profit organization that provides no-cost health services to students of all ages. This position will rotate between the Bear Care Clinic, located in the White Bear Lake Area School District and the St. Paul City School mobile clinic. Rise Up Health Clinics operates two of Minnesota's twenty-nine school-based health clinics and has Minnesota Essential Community Provider designation.


The candidate must be flexible in their ability to work one or two days per week (Mondays and/or Tuesdays) and will report to the Medical Director and Clinic Director, and. The candidate will also help with mobile clinic events and provide direct patient care and daily clinic operation assistance.


The position is a joint-appointment model with St. Catherine University; the practitioner will be hired as an adjunct faculty for St. Catherine University, with the primary responsibility of working in the Bear Care Clinic and St. Paul Clinic and precepting students. There are no benefits with this part-time position.


NURSE PRACTITIONER

The nurse practitioner will have the ability to engage with students and patients from diverse cultures, possess excellent communication skills, and demonstrate a strong foundation in pediatric and adolescent care. This role requires the ability to practice independently while collaborating effectively with clinic staff.


Responsibilities:

  • Conduct assessments, diagnose illnesses, prescribe medications and treatments, provide patient education, and make referrals for additional services as needed.
  • Develop and implement evidence-based individual treatment plans to ensure comprehensive and effective patient care.
  • Maintain compliance with HIPAA regulations to safeguard patient privacy and confidentiality.
  • Obtain comprehensive health histories through patient and family interviews, assess presenting illnesses and risk factors, review family health histories, and gather psychosocial, developmental, and cultural information.
  • Assess the patient or family’s ability to access and afford care and prescribed treatments. Utilize resources to make care more accessible to patients. 
  • Collaborate with clinic staff to optimize patient workflow, ensure efficient clinic operations, and deliver high-quality, evidence-based care.
  • Uphold professional, legal, and ethical standards in practice through participation in continuing education and annual education requirements.
  • Coordinate patient care with mental health providers, health educators, nutritionists, and students' primary care providers as necessary.
  • Serve as a preceptor for multiple students and actively participate in community outreach initiatives.
  • Participate in mobile health clinics as required to meet community healthcare needs.
  • Collaborate in developing clinic policies and processes, demonstrating a willingness to contribute to continuous improvement.
  • Attend regular staff and provider meetings to facilitate communication and contribute to clinic operations.
  • Ensure that care provided is culturally responsive and gender-affirming, promoting diversity and health equity. Engage and participate in DEI education and training.


Qualifications:

  • Master’s degree or higher as a Pediatric or Family Nurse Practitioner.
  • Active Minnesota State license as an Advanced Registered Nurse Practitioner (ARNP).
  • One year of professional work experience as a nurse practitioner
  • Commitment to School-Based Health Centers (SBHCs), diversity, and health equity.
  • Exceptional interpersonal communication skills with a focus on patient-centered care.
  • Motivated to improve clinic accessibility and service delivery to meet patient needs.
  • Highly organized with the ability to work independently and collaboratively within a team.
  • Previous experience in School-Based Health Center (SBHC) clinics is preferred.
  • Proficient in implementing evidence-based practices and utilizing technology for healthcare delivery.
  • Comfortable with learning new software, technology, and virtual programs to enhance patient care and clinic operations.
