Since 1985, BHCHP’s mission has been to ensure unconditionally equitable and dignified access to the highest quality health care for all individuals and families experiencing homelessness in greater Boston. Over 10,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. Job Summary:

Come join Boston Health Care for the Homeless Program (BHCHP) as we build an exceptional care team that takes care of individuals experiencing homelessness

You will be a member of a multi-disciplinary clinical team consisting of nurses, a recovery coach, physicians, psychiatrists, NP/PAs, case managers and mental health clinicians. The Street Team provides primary and continuous medical and psychiatric care to individuals currently or with a history of living on the streets of Boston. The service delivery model includes bringing medical and psychiatric care directly to the streets and to patient’s homes as well as caring for patients in BHCHP’s MGH clinic. We also visit patients when they are admitted to nursing homes and area hospitals when appropriate.

Responsibilities:

  • Provides comprehensive, high-quality primary, and/or urgent care to adults who are currently or have experienced homelessness
  • Elicit an appropriate history, perform physical examinations, order diagnostic studies, interpret results and initiate therapy in accordance with The Guidelines for Physician Assistants and Nurse Practitioners of Boston Health Care for the Homeless Program
  • Refer patients as appropriate to medical specialists, hospital admission, medical respite care and detox facilities, providing appropriate information for continuity of care
  • Join the Street Team members to conduct medical care on the streets, in drop-in shelter clinics, at hospital based BHCHP clinics, and in patients’ homes
  • Participate in collaborative relationship with BHCHP physician according to the Regulations of the Commonwealth of Massachusetts
  • Provide on-call coverage with other BHCHP clinicians to ensure 24-hour availability and to provide onsite holiday coverage at the respite facilities
  • Provide education to patients on health promotion and disease treatment and prevention
  • Participate in peer education by giving conferences in areas of interest and expertise; participate in the teaching (formal and informal) of medical students, nurse practitioner students and physician assistant students
  • Participate and contribute to department, general staff, discipline specific and committee meetings; participate in at least 1 equity program each year
  • Maintains and enhances professional knowledge by attending CME conferences, educational programs and seminars according to licensure requirements and to maintain the highest level of clinical competence; participate in quality assurance activities
  • Maintain adequate, up-to-date medical records, completed in a timely manner, according to the guidelines of BHCHP, employing standard forms such as problem lists, medication lists, etc
  • Maintain active DPH, DEA, and professional license, hospital privileges, managed care credentialing and CPR training; maintain clinical privileges and credentials at MGH

Qualifications:

  • Licensed by the Commonwealth of Massachusetts
  • Registered or eligible to be registered with the Massachusetts Department of Public Health and the Federal Drug Enforcement Agency
  • Bilingual preferred
  • Experience in working with underserved populations, mentally ill, homeless and/or persons with substance abuse problems preferred.

Compensation and Benefits:

BHCHP full time employees are eligible for our competitive time off policy of 4 weeks’ vacation, health, dental and vision insurance, 403B retirement savings plan and employer retirement contribution, and pre-tax MBTA pass program with 40% discount. In addition, eligible employees will receive yearly increases, additional compensation of seven thousand five hundred dollars added to your base hiring rate for demonstrated bilingual proficiency and the opportunity to work with local hospitals and community health centers.
