Provide personal care services under the direction of the Registered Nurse or Therapist.

The Home Health Aide is assigned to specific clients by the Registered Nurse or other appropriate professional and performs services for clients as necessary to maintain their personal comfort. Written instructions for patient care are prepared by a Registered Nurse or Therapist.

SPECIAL INSTRUCTIONS

The Registered Nurse or Therapist shall make a supervisory visit to the patient’s residence at least every fourteen (14) days either when the Home Health Aide is present to observe and assist or when the Home Health Aide is absent to assess relationships and determine whether goals are being met.

DUTIES AND RESPONSIBILITIES

1. Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist. a. Range of motion exercises and positioning b. Assistance in ambulation or exercises c. Safe transfer techniques

2. Performs personal care activities, including but not limited to: a. Bed bath b. Shampooing – sink, tub, or bed c. Sponge, tub, or shower bath d. Skin care/nail care e. Oral hygiene f. Shaving g. Dressing h. Toileting and elimination

3. Performs household services essential to health care at home, including but not limited to: a. Meal preparation with adequate nutrition and fluid intake considerations/feeding b. Laundry c. Light housekeeping

4. Reports any observed or reported changes in the client’s condition and/or needs to the Registered Nurse or Therapist. Follows reporting guidelines in the Care Plan.

5. Documents care provided and pertinent observations. Completes the appropriate records required at the time of visit. Records documentation the day of the visit. Documentation includes but is not limited to: a. Reading of temperature, pulse, respiration, and blood pressure b. Basic elements and changes in body function

6. Promotes personal safety and a safe and healthy environment for clients by observing infection control practices, following Agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager.

7. Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs.

8. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings and team conferences as requested.

9. Recognizes the physical, emotional, and developmental needs of and ways to work with the populations served by the Agency, including the need for respect for the patient, his or her privacy and property.

10. Maintains confidentiality in all aspects of the job.

11. Attends in-service programs to meet compliance requirements. Identifies learning needs to Agency Supervisor.

12. Performs other related duties and responsibilities as deemed necessary.

COMPENSATION:
$30 per visit and mileage 0.625 cents/mile
plus intermittent visits.
