*Job Summary*
We are seeking a dedicated and knowledgeable Physical Therapist to join our healthcare team. The ideal candidate will be responsible for assessing, planning, and implementing treatment programs for patients to improve their physical function and mobility. This role requires a strong understanding of anatomy and medical terminology, as well as experience in various therapeutic techniques, including trigger point therapy and discharge planning. The Physical Therapist will work in both inpatient and outpatient settings, providing care plans tailored to each patient‘s needs.

*Responsibilities*
* Conduct thorough assessments of patients‘ physical abilities and limitations.
* Develop individualized treatment plans based on patient evaluations and goals.
* Implement therapeutic interventions to enhance mobility, strength, and overall physical function.
* Utilize techniques such as trigger point therapy to address specific pain points.
* Collaborate with other healthcare professionals to ensure comprehensive patient care.
* Educate patients and their families on home community care practices and exercises.
* Monitor patient progress and adjust treatment plans as necessary.
* Maintain accurate documentation of patient evaluations, treatments, and outcomes.
* Participate in discharge planning to facilitate smooth transitions from inpatient care to home health or outpatient services.

*Requirements*
* Valid state licensure as a Physical Therapist.
* Strong knowledge of anatomy, medical terminology, and physical therapy techniques.
* Experience in inpatient settings preferred; familiarity with home health care is a plus.
* Ability to develop effective care plans tailored to individual patient needs.
* Excellent communication skills for educating patients and collaborating with healthcare teams.
* Commitment to ongoing professional development and staying current with industry best practices.

Pay: $45.00 - $47.00 per hour
