In every interaction, no matter how big or small, we're dedicated to providing a superior experience for patients facing life-limiting illnesses and their families.Qualifications:A heart to serve patients and families and a passion for providing the best possible careEducation:Graduate of an accredited masters, post masters, or doctoral nursing program with formal preparation as a Nurse Practitioner.Licensure:Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner. Current prescriptive authority with full DEA privileges. Must achieve and maintain national certification in the Nurse Practitioner specialtyExperience:2 years experience within the last 5 years in chronic disease management/long-term care.Hospice and/or Palliative background preferred.Required:Reliable transportation.
