Key Responsibilities:

Clinic Operations:

  • Ensure the clinic is high-performing, well-equipped, and safe.
  • Maintain workflow efficiency and clarity, and enforce protocols.

Staff Management:

  • Hire, retain, coach, develop, and supervise RN Case Managers, Clinic RNs, Medical Assistants, and other clinic team members.
  • Ensure staff engagement and well-being.

Data Analysis and Improvement:

  • Analyze data for trends, strengths, weaknesses, and opportunities.
  • Perform root cause analysis and evaluate care delivery processes.
  • Recommend strategy, workflow, and policy changes to the CSD to improve clinic operations.

Care Coordination:

  • Ensure nursing and clinic staff deliver and document interventions as outlined in participants’ care plans.
  • Respond promptly and accurately to physician orders and complete assessments.
  • Participate actively in care planning for assigned participants.

Quality and Compliance:

  • Evaluate the quality and effectiveness of services in conjunction with medical practice leadership and other relevant disciplines.
  • Collaborate with the CSD and Quality Improvement Director to lead clinical and nursing audits during CMS/DHCS reviews.
  • Develop and monitor corrective action plans to ensure ongoing compliance.

Job Requirements:

Education and Licensing:

  • Bachelor’s degree in nursing or a related healthcare field preferred; relevant professional experience may substitute.
  • Valid registered nurse (RN) state license required.

Experience:

  • Minimum of two years of experience managing an ambulatory clinic operation, including oversight of RN, LVN, and MA staff; equivalent nurse leadership experience in a high-volume outpatient setting may substitute.
  • Minimum of two years of clinical experience with a frail or elderly population.
