Join our Team as an RN Case Manager FT
- Position Summary:
- Responsible for coordinating care efficiently and effectively to help patients achieve optimal health and appropriate resource utilization.
- Ensures care is provided at the right level per medical necessity, assesses patient transition needs for timely discharge and readmission prevention.
- Integrates national case management standards including Utilization Management, Transition Management, Care Coordination, and Compliance with regulations and policies.
- Facilitates patient care conferences, performs accurate medical necessity screenings, leads case reviews, and collaborates with healthcare teams for holistic patient care.
Qualifications:
- Education
Required: Graduate of an accredited nursing school
Preferred: Bachelor's or Master's degree in nursing - Experience
Required: 2 years of acute hospital patient care
Preferred: Acute hospital case management - Certifications:
Required: Active RN license
Preferred: Accredited Case Manager (ACM)
