Job Purpose and Focus:
  • Responsible for the planning, coordination, family support/referral/intervention, record keeping, documentation and overall direction for all Early Head Start program activities related to the provision of home-based services
  • Responsible for building the key relationships between the enrolled family and the program

Essential Qualifications and Requirements
  • BA/BS in Early Childhood Development
  • Associate degree in Early childhood Development or related field
  • CDA in home visiting or related field

Essential Functions and Representative Duties
  • Primary responsibility for sustaining a positive and effective mentor/trust position with each family that nurtures maternal-infant/toddler bonding and supports the health and development of the parent and child.
  • Primary responsibility for linking maternal/infant-toddler/family needs with available resources withing the local programming and or community
  • Ensure that the homebased development/educational program integrates all required components in accordance with the federal performance standards, best practices of infant/toddler and maternal support.
  • Provide home-visits of required duration and frequency to meet family needs and fulfill requirements of the EHS grant.
  • Maintain a working knowledge of the federal performance standards especially related to child development and its integration within the Carman Ainsworth Early childhood Curriculum and philosophy; and all assessment tools and systems utilized within the CAEHS program.
  • Maintain primary responsibility for the effective involvement of the CAEHS center- based staff, other caregivers serving families, program coordinators and relevant community partners as appropriate to support the growth development health and well-being of caseload families most effectively.
  • Develop a family partnership agreement with each caseload family, ensuring ongoing review and support for participant-defined personal and family goals.
  • Arrange in cooperation with the Child Development/disabilities/nutrition/ health/ family engagement and behavioral health coordinators as appropriate, for the in-depth evaluation or assessment of children having suspected special needs, problems or handicapping conditions; and participate in the IFSP/IEP conference as appropriate.
  • Implement home-based services of the IEP and or IFSP as outlined in the documents and/or as recommended by the child development /disabilities-nutrition/health/ family engagement/behavioral health coordinators as appropriate
  • Participate in at least monthly reflective supervision with the CAEHS Director/Behavioral Health/Child Development Coordinator or Coach Mentor.
  • Maintain ongoing record keeping of home-based services including documentation of visits, content of visits, assessments related to growth and development, family referrals/needs/ follow-up.
  • Ensure that each child and parent on the caseload is up-to-date on all necessary health screenings, immunizations and well-baby visits.
  • Attend all staff meetings, professional development and other scheduled program activities relevant to home-based services.
  • Participate in the ongoing monitoring and assessment of the entire program and provide input for continuous improvement.
  • Complete and submit all paperwork, reports and records on time and in a professional manner.
  • Make parent and or child referrals according to standard program procedures and within a timely fashion to the Child Development/Disabilities-Nutrition/Health/Family Engagement or Behavioral Health Coordinators.
  • Maintain educational and health credentials and files as necessary.
  • Other related duties as required and or requested.

Terms of Employment
Salary according to Carman-Ainsworth Federal Programs Salary Scale; non-bargaining unit position. Pay rate based on years of experience and education. Increments based on EHS pay scale and grant funding availability. This is a 48 week position for 20 hours/week, mileage reimbursement at federal rate.

Evaluation/Performance Measurements
Ongoing self assessment and supervision with Health and Family Support Coordinator and/or Child Development Coordinator will be maintained on the established program assessment form. Individual supervision and support will be provided as specific professional needs are identified and documented within the self assessment process. Individual training will be provided as specific professional development needs are identified and documented within the professional development Plan incorporated in the self assessment process.
