**Are you passionate about making a difference in the lives of those facing advanced illnesses? Our Hospice Team is seeking a dedicated RN, Case Manager to join our compassionate team.
*What You’ll Do:*
* *Patient Assessment*: Conduct thorough assessments of patients' physical, emotional, and spiritual needs.
* *Care Planning*: Develop individualized care plans in collaboration with the interdisciplinary team and family.
* *Coordination of Care*: Coordinate services among healthcare providers, caregivers, and community resources.
* *Patient and Family Education*: Provide education on disease progression, symptom management, and available hospice services.
* *Symptom Management*: Monitor and manage symptoms to enhance patient comfort, including pain control and other interventions.
* *Advocacy*: Advocate for the patient’s wishes and needs, ensuring they align with their goals of care.
* *Communication*: Maintain clear communication with patients, families, and the healthcare team regarding changes in the patient’s condition.
* *Emotional Support*: Offer emotional and psychological support to patients and their families during the end-of-life process.
* *Documentation*: Maintain accurate and timely documentation of care plans, assessments, and patient progress.
* *Crisis Management*: Respond to urgent needs and crises as they arise, providing timely interventions.
* *Quality Improvement*: Participate in quality improvement initiatives to enhance hospice care services.
* *Training and Mentoring*: Provide guidance and support to new staff and volunteers in hospice care practices.
