As a Physical Therapist in Home Health, you will assess and evaluate patient care needs focusing on functional status, mobility, endurance, pain, and other medical conditions. Your expertise will shape treatment plans aimed at enhancing patient well-being while tracking their progress. Key Responsibilities: Evaluate patients’ functional status and physical therapy needs, assisting physicians in therapy plan development. Provide services in accordance with state laws and the established plan of care, collaborating with the health care team. Observe, document, and communicate the patient's response to treatment and any changes in condition. Deliver treatments to relieve pain and restore function, adhering to accepted professional standards. Educate the health care team, patients, and their families regarding the therapy plan, functional limitations, and progress. Visit patients per the treatment plan, complete progress notes for each visit, and submit notes according to LHC policy. Conduct comprehensive patient evaluations and develop the PT plan of care within state guidelines. Report evaluation outcomes and projected frequency of care. Required Qualifications: Active Physical Therapy licensure in the relevant state. Current CPR certification. Valid driver's license and vehicle insurance, or access to reliable transportation. Ability to work in diverse home settings regardless of the client’s background or circumstances. Compensation is based on various factors, including local market conditions, experience, and qualifications. Our pay rates range from $34.23 to $61.15 per hour for full-time employment.
