Job Summary:

We are seeking a Registered Nurse (RN) Case Manager in the Bridgewater/Brockton/Stoughton areas. This is a field-based position providing 1:1 end-of-life care. The ideal RN is a team player, possesses leadership in patient care, is a problem solver within a team, and helps patients and their families during trying times. The goal is to promote patients and their families by providing high-quality hospice care.

Responsibilities:

Provide skilled nursing and head-to-toe assessments to the adult and geriatric population. This includes the oversight of medication administration and wound care treatment as prescribed in the physician's plan of care.
Provide visiting hospice care services to an assigned caseload including admissions, re-certification and discharges.
Develop the Plan of Care in coordination with all Hospice care professionals assigned to the case.
Be knowledgeable in the use of the current assessments for all patient admissions, re-certification, transfers and discharges.
Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
Report to the Clinical Manager on a daily basis.
Provide documentation using the provided iPad within the required time frame.

Qualifications:

Licensed Registered Nurse (RN) in the state of Massachusetts with at least 1 year of clinical experience; hospice care expereince preferred.

Have reliable transportation and be willing to commute to provide the best in-hospice patient care.
Basic computer skills, excellent documentation and customer service skills.
Knowledgeable regarding end-of-life care and death and dying process.
