If that sounds like a fit, we'd love to hear from you.Responsibilities•Complete in-home assessments and manage ongoing symptom control•Create and update care plans based on patient needs•Provide hands-on clinical care with a focus on comfort and dignity•Work closely with our medical director, social workers, and aides to deliver coordinated care•Support families emotionally and help them understand the hospice journey•Advocate for patients' wishes and cultural values•Attend team meetings and collaborate on care strategies•Communicate with outside providers when needed•Educate families on what to expect and how to support their loved one•Keep thorough and timely charting for compliance and continuity of careQualifications•Active RN license in New Jersey•2-3+ years of hands-on nursing experience (hospice or palliative care is a big plus)•Strong understanding of end-of-life care and the hospice model•Compassionate communicator - with patients, families, and your team•Emotionally grounded, with the ability to support others through tough moments•Comfortable working independently in the field•Reliable with time management and documentation•Familiar with EMR systems and basic charting tools
