Location: Community Care Hazleton Shift: Days (United States of America) Scheduled Weekly Hours: 40 Worker Type: Regular Exemption Status: No Job Summary: Acts as the coordinator within Community Care to identify appropriate patients and opportunities regarding scheduling, documentation and plans of care to support health management. Collaborates with patients/families/legal representatives, at multiple Community Care sites, to facilitate clinical guideline adherence and coordinates services specifically related to the patient's health care needs. Functions as the main facilitator for conducting the entire process of Diagnostic Tracking and Referral Tracking as required by HRSA and outlined in the HRSA Compliance Manual. Serves as a vital participant for tracking or follow up activities related to projects implemented from Federal or State grant awards for FQHC LAL Job Duties: Collaborates with Providers and the entire health care team to enhance clinical guideline adherence and promote best practice. Evaluates the need to secure Referrals by utilizing the current processes of notifying patients of the elapsed referral and potentially assist with scheduling. Notifies patients of the elapsed, ordered diagnostic and laboratory studies by utilizing the current process of notification. Directs, plans, and implements improvement plans for specific chronic disease management and quality focus. Performs regular on-site visits to review/evaluate outcomes for these programs according to Standards of Care. Identifies problems and proposes strategies for quality improvement. Analyzes, plans, implements, and evaluates documentation in the EMR, assuring that documentation meets chronic disease management and quality metrics/guidelines. Performs other duties as required or assigned, for operational reasons of which the employee is qualified to perform. Maintains patient confidentiality and protects Protected Health Information (PHI), with all activities related to this position.
