Speech Language Pathologist
POSITION SUMMARY:
You will make home visits to assess and treat speech, language, social communication, cognitive communication, and swallowing disorders. You‘ll be responsible for providing detailed notes and documentation regarding your home therapy visits and patient progress.
RESPONSIBILITIES:
Knowledge of the principles and practices of speech pathology and provides speech therapy services in accordance with state practice act
Perform an initial assessment and evaluation of the patient and establish a plan of care, including scheduling subsequent visits determined by the SLP to be clinically appropriate.
Coordination of care by care conferencing with other healthcare team members
Observe, record, and report to the Primary Care Physician the patient’s treatment, response, and progress including significant changes
Obtain orders to revise the plan of care as clinically appropriate
Perform re-evaluations of the patient’s progress at least every thirty (30) days and update the plan of care as necessary
Complete comprehensive assessments and OASIS data as necessary
Teach patients and caregivers about the patient‘s impairments and provide support and educational materials to support interventions and foster independence
QUALIFICATIONS:
Master‘s degree in Speech-Language Pathology
Active Speech Language Pathology license (CCCs)
Ability to practice speech therapy unencumbered by state and federal guidelines
At least one year of experience working as a Speech Language Pathologist (no CFYs)
Home Health and Home Care Home Base documentation experience
