*Overview*
We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will possess a strong background in patient care, with experience in both outpatient and inpatient settings. This role requires a commitment to improving the quality of life for patients through effective rehabilitation strategies and personalized treatment plans. The Physical Therapist will work closely with patients to assess their needs, develop tailored therapy programs, and support them throughout their recovery journey.

*Duties*
* Conduct thorough assessments of patients' physical abilities and limitations.
* Develop individualized treatment plans based on patient evaluations and goals.
* Implement therapeutic exercises and interventions to improve mobility, strength, and overall function.
* Provide education to patients and their families regarding treatment protocols and home care strategies.
* Monitor patient progress and adjust treatment plans as necessary.
* Collaborate with other healthcare professionals to ensure comprehensive patient care.
* Participate in discharge planning to facilitate smooth transitions from hospital or outpatient settings to home or community care.
* Utilize techniques such as trigger point therapy as part of the treatment process.

*Skills*
* Strong knowledge of physiology and anatomy to inform treatment decisions.
* Proficient in patient care practices across various settings including home health, outpatient clinics, hospitals, and inpatient facilities.
* Experience with discharge planning processes to ensure continuity of care.
* Ability to develop rapport with patients and motivate them throughout their rehabilitation journey.
* Excellent communication skills for effective collaboration with patients, families, and interdisciplinary teams.
* Familiarity with evidence-based practices in physical therapy, including various therapeutic modalities.

Job Types: Full-time, Contract

Pay: $43.00 - $45.00 per hour

Expected hours: 40 per week
