Client location and schedule hours:

Client located in the Franklin area. Schedule is M-F 10 am to 6 pm
Job Responsibilities:

There are no pets in the home.  Client does not drive now but has a car in the garage for CG to use if needed. Her sleeping and bathing quarters are on the main floor of their home. Client  is very independent-minded and described by family as blunt or direct way of communication.

Diagnoses:  Vascular Dementia, Post TIA's (4 separate ones in all different areas), Incontinence issues, 2 leaky heart valves (one repaired), Post small cell lung cancer 2004 (in remission). Recent hospitalization for a UTI and systemic infection No Hearing Loss


Allergies:   Food: None known    Med allergies:  Penicillin, Codeine

Medications:  Aspirin 81mg /day, Lipitor daily, Protonix daily & others (see discharge list)

Goals & Objectives:   Keep patient clean & dry, changing Depends as needed. Offer shower assistance or bed bath if too weak. Offer to assist with dressing. Prepare/warm up food for meals or snacks & serve. Offer fluids often. Encourage and assist client with exercises recommended by PT or Dr. 2x per shift to help regain her strength. Engage with to gain trust. Light housekeeping (clean kitchen/bathroom), do laundry & change her bed linens as needed. May drive or accompany to appts or run errands.

ADL's:


Mobility:  Client  is weight-bearing but now needs assistance on all transfers and should be observed for imbalance. Client uses a walker or transport chair and is getting PT to regain strength. She resides on the main floor of her home but has a stair lift chair to get downstairs to her car if needing to get into her car to go somewhere. 

Eating:  Client  is a good eater with no food allergies known. She does not wear dentures. She needs meals prepared and served to her but she can feed herself and may want to decide what she would like to eat. No special diet but salt restricted. She needs to be reminded to stay hydrated. 

Toileting:  Client  will need transfer assistance but likes her privacy once there. She may need help wiping but ask first. She has been putting on her own depends but ask if she needs help due to recent weakness. She usually knows when she needs to go but has been having bowel movements in her depends while in hospital. 

Bathing/Dressing/Grooming:  Client  normally likes to bathe daily but goal is every other day (MWF). She has a nice (small) step-in shower area with bath chair and 3 support bars, with a hand help shower head she normally uses. She needs help getting into the shower and out and may want help drying off or washing her hair 2x per week. She picks out her own clothes. Client does not have dentures and can brush her own teeth if given toothbrush w/paste. She may want to try to wear makeup or get assistance.

Communication:  Client  is from Brooklyn, NY & has been described as having a " direct" way of communicating. They say she has a little paranoia associated with her dementia. She has some difficulty producing the words she wants to say and does have a blank look at times. She may repeat herself or ask a lot of questions. Her speech, however, is clear and she can make her own needs known. 

Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language 
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
TB test
