About Position Overview:


Justice Assistance Health & Wellness is seeking a compassionate and dedicated Nurse Practitioner to join our new health and wellness clinic to provide care consistent with the philosophy of Justice Assistance. The position would start on a part-time basis with the potential to grow to full time. The ideal candidate will provide high-quality care to our patients, focusing on primary care, mental health assessment and/or medication evaluations, diagnosis, and treatment. As our nurse practitioner, you will be part of an integrated team of Behavioral Health clinicians and work with our Chief Medical Officer.  This position offers a supportive work environment, flexible hours, and the opportunity to make a meaningful impact in the lives of individuals.  We are committed to creating an inclusive and supportive environment for all employees, clients and patients. Key Responsibilities:

  • Patient Care: Provide primary care to members, including histories, exams, lab tests, diagnostics, and referrals as well as conduct comprehensive mental health assessments.
  • Diagnosis and Treatment: Develop and implement individualized treatment plans, including medication management and therapeutic interventions.
  • Patient Education: Educate patients and their families about medical and/or mental health conditions, treatment options, and preventive measures.
  • Collaboration: Work collaboratively with psychiatrists, psychologists, therapists, and other healthcare professionals to ensure coordinated care.
  • Documentation: Maintain accurate and up-to-date patient records using an electronic health record (EHR), including progress notes, treatment plans, and any changes in condition.
  • Follow-Up: Provide ongoing monitoring and follow-up care, adjusting treatment plans as necessary to ensure optimal patient outcomes.
  • Crisis Intervention: Address acute mental health crises and provide immediate, effective interventions.


Qualifications:

  • Education: Master’s or Doctoral degree in Nursing from an accredited program with a specialization in psychiatry or mental health.
  • Licensure: Current Nurse Practitioner license in RI with prescriptive authority.
  • Certification: Board certification as a Family/ FNP and/or Psychiatric Mental Health Nurse Practitioner/PPHNP.
  • Experience: Minimum of [2-5] years of clinical experience in a mental health setting.
  • Ongoing CEU to update skills/therapies
  • Skills: Strong clinical assessment and diagnostic skills, excellent communication and interpersonal skills, ability to work independently and as part of a team.
  • Attributes: Empathy, patience, and a genuine passion for providing care to a diverse patient population.
  • Ongoing CEU to update skills/therapies


Scope of Work, Schedule and Hours:

  • Be able to see 6-12 patients per day and complete accurate documentation within 48 hours of visit into electronic health record system.
  • Works as part of an integrated medical team by assisting others in their roles as providers to ensure integrated and continuous care.
  • Provide comprehensive medical services to all patients without bias for age, sex, gender identification, or criminal history.
  • Consider patients’ cultural, psychological, and social and environmental factors.
